im_narrator provides text-to-speech and speech-to-text services for Instant Messaging clients for the blind (and others). It currently supports AIM, MSN, ICQ, Exodus, Jabber Messenger, and Meca on win32, and Adium X on Mac OS X. Ports to other platforms are planned. Python source code is available via CVS. Binaries for win32 are available from the downloads page.
We're actively seeking ports to other platforms and other IM clients. If you want to try and add support for your favorite IM client, see these instructions.
